Consumer Access To Records and Complaints

  • You have the right to access, inspect, and obtain a copy of your Protected Health Information (PHI). You can request copies of your PHI by sending a written request to the office address below.

    When sending a request, please include your full name, date of birth, and specific records you are requesting. Requests may be subject to identity verification.

    In most cases, a copy of your PHI will be mailed to you within fifteen (15) business days. Radiant Light Counseling Services, PLLC may charge a reasonable fee for copying, mailing, and other supplies necessary to fulfill your request.

    If records are requested on behalf of another individual, documentation verifying legal authority is required. Examples include guardianship documentation or medical power of attorney.

  • You have a right to have your complaints heard and resolved in a timely manner. If we cannot work things out to your satisfaction, you may file a complaint with your counselor’s licensing board.
